I. Introduction

You are reading this Telehealth Informed Consent as part of your use of MedRising’s telehealth services provided through our platform. By using MedRising’s website, mobile apps, or other telehealth technologies (collectively, the “MedRising Platform”), you are consenting to telehealth services offered by our affiliated healthcare providers.

When you create an account, start a consultation, or click “I consent to telehealth,” you acknowledge that you have reviewed this document and consent to receive telehealth services through MedRising. A record of your consent is kept in the provider’s files, and each telehealth session you engage in reaffirms your ongoing acceptance of this consent.

II. What is Telehealth?

Telehealth involves delivering healthcare services through electronic communication between a healthcare professional and a patient in different locations. These services may include diagnosis, treatment, follow-up care, and education via video, audio, and store-and-forward methods. This document details the benefits, risks, and limitations of telehealth to ensure informed participation.

III. Benefits of Telehealth

Telehealth offers the benefit of convenient, flexible access to healthcare services without an in-office visit. You may access follow-up care or receive health guidance at times that suit your schedule. IV. Possible Risks of Telehealth

While telehealth is a powerful tool, it has limitations:

  • Transmitted information may be incomplete, impacting the quality of healthcare you receive.

  • Technical issues can delay or prevent service delivery.

  • A lack of complete medical records may increase the risk of adverse interactions or misjudgments.

Should technical issues occur, you may contact us at 1-800-909-787-2342 during the same hours noted above. You may discontinue telehealth services at any time, but fees may apply for completed consultations.

MedRising may also offer diagnostic testing services through third-party labs, whose accuracy we cannot guarantee. These tests may produce false results, impacting the effectiveness of your care.

V. Patient Acknowledgments

  4. You understand that your provider may be a nurse practitioner or physician assistant.

  5. You will be informed if others are present during a telehealth visit.

  6. You affirm that the information you provide is accurate and complete.

  7. Your provider may choose to provide care asynchronously through store-and-forward technology if deemed appropriate.

  8. You understand there is no guarantee of receiving a prescription.

  9. You agree to hold MedRising and its affiliates harmless for delays or issues due to technical failures.

  10. Diagnostic services offered may contain defects that could affect the care you receive.

  11. The MedRising Platform is limited to a specific set of healthcare services, and your provider may deny care if it is deemed clinically inappropriate.

  12. Telehealth consultations may not always be in real time, so you should regularly check the MedRising Platform for updates.

  13. You have the right to discuss telehealth services, benefits, risks, and alternatives with your provider.

  14. You have access to your medical records and may share them with other healthcare providers as needed.

  15. You may request to use a pharmacy or lab of your choice.

  16. MedRising has rights as a third-party beneficiary of this consent.
